Step One
Fill out the Online Intake Form with as much detailed information
as possible.
Step Two
Submit Initial Intake Fee via PayPal.
Step Three
Email us a jpg file of your animal or mail us a 4x6 photo of
your animal.
Step Four
Once we received your Online Intake Form, PayPal approved payment,
and picture of your animal, we will contact you by phone to set up your
intake and treatment appointment.
